Nicholas I Cole, Pauline A Swift, Rebecca J Suckling, Peter A Andrews South West Thames Renal Department, Epsom and St Helier University Hospitals, Surrey, UK Address for correspondence: Dr Nicholas Cole Renal Department, St Helier Hospital, Wrythe Lane, Carshalton, Surrey SM5 1AA, UK Tel: 44 (0)7758217166 Fax: 44 (0)2082962857 E-mail: [email protected] Type 2 diabetes mellitus and chronic kidney disease (CKD) frequently co-exist and the increasing burden of both conditions is a global concern. Metformin is established as the first-line treatment for type 2 diabetes because it is associated with improved cardiovascular outcomes and a reduced risk of hypoglycaemia compared with other treatment options. Patients with CKD may benefit in particular because they are at high risk of both cardiovascular disease and hypoglycaemic episodes. However, the use of metformin is restricted in this population due to the concerns over lactic acidosis. Recent reviews have evaluated this risk and concluded that current guidelines for prescribing metformin in CKD may be too restrictive. This narrative review considers this evidence further, but also examines the strength of evidence that favours the use of metformin in CKD patients. 2016;8-175 Key words: type 2 diabetes mellitus, chronic kidney disease, metformin, biguanides, lactic acidosis, lactate, cardiovascular disease, hypoglycaemia Chronic kidney disease (CKD) commonly co-exists with diabetes mellitus; the estimated prevalence of Kidney Disease Outcomes Quality Initiative (KDOQI) stage 3–5 CKD in the UK for those with diabetes is 31%. Actually, metformin is usually not the original cause of kidney problems. However, metformin is eliminated by the kidneys and when a patient has poor. The revised guidelines stated that the use of metformin is only absolutely contraindicated in patients with severe chronic kidney disease CKD eGFR 30. To the Editor In their article about the care of patients with advanced chronic kidney disease, Sakhuja et al1 mentioned that metformin is contraindicated in.

Metformin is one of the oldest and most widely prescribed antidiabetic medicines worldwide. It is the only such medicine that has shown a reduction of cardiovascular mortality in diabetes mellitus type 2. Since many diabetic patients have chronic kidney disease, its use is often curtailed by practitioners due to fear of lactic acidosis and the US Food and Drug Administration (FDA) warnings that, until recently, had been in place for decades. Current guidelines, though somewhat vague regarding dosages, clearly pave the way for spreading the use of metformin in patients with lower glomerular filtration rates. These guidelines also suggest moving away from just looking at serum creatinine to create a cut-off. Metformin’s costs are lower, and in many underdeveloped countries this is the only medicine available for poor patients. More widespread use of metformin will further help with health care costs, as well as obesity. Metformin is a prescription drug used to treat type 2 diabetes. It belongs to a class of medications called biguanides. People with type 2 diabetes have blood sugar (glucose) levels that rise higher than normal. Instead, it helps lower your blood sugar levels to a safe range. This may make you wonder what side effects it can cause. Metformin can cause mild and serious side effects, which are the same in men and women. Here’s what you need to know about these side effects and when you should call your doctor. Find out: Can metformin be used to treat type 1 diabetes? These can occur when you first start taking metformin, but usually go away over time. Tell your doctor if any of these symptoms are severe or cause a problem for you.

Contrary to popular conception, Viagra is NOT an aphrodisiac. Viagra does nothing to intensify the sensations or feelings that occur during sexual intercourse or masturbation, it merely facilitates a stronger, more firm erection in adult men who have difficulty maintaining one normally. Given the propensity of adolescent boys to experience erections at the least provocation, there is little that a drug like Viagra can offer to a 13 year old. It is a bit like ligthing a match on the surface of the sun. Young men and adults who have not been prescribed Viagra by a doctor, should avoid using the drug because it puts them at risk for a serious side effect called "Priapism", which is a prolonged erection over a period greater than four hours. All potential bad jokes and kidding aside, an erection that goes on for more than four hours will become very painful, and a condition called "ischemia" or "penile fibrosis" may result from the scarring of the spongy, erectile tissue that forms the penis. Priapism is a medical emergency that must be treated fairly rapidly, and is primarily treated through injections of alpha-adrenergic drugs directly into the penis, or cavernosal phlebotomy (removal of the blood from the cavernousal bodies of the penis, with a needle). The very treatment of priapism itself may lead to complications like hematoma (swelling due to fluids infiltrating the tissues), bruising and chronic impotence.
